Please complete the form below to replace your defective item as per the Warranty guidelines. Please submit one form for each item.
If the item being replaced is listed in the drop-down box, then you do not need to return the defective item. We will automatically send a replacement item to you.
If your item is not listed, then select “Other” and provide us with as much information as possible regarding the defect and the item in the Product Description area.
After you submit the form, print out 2 copies of the confirmation page, include one copy with the shipment of the item to us and keep one copy for your records. Make sure to retain the “Return Request ID” for your records. You will need it when inquiring about the status of your return.
